“A lot of our patients are dealing with untreated mental health conditions that affect their ability to stay housed, get food, or manage chronic illnesses,” Dr. Delong explained. “By providing integrated behavioral and medical care, we’re supporting them in stabilizing their health and their lives.”

The behavioral health team doesn’t just serve patients on-site. They’re embedded across multiple locations—shelters, outreach efforts, and more—ensuring consistent care for individuals wherever they are in their journey.
