Receive considerate, respectful care
You will be treated with dignity at all times. We provide services to all patients—regardless of race, nationality, sex, age, sexual orientation, gender identity, genetic information, military or veteran status, disability, diagnosis, or religious beliefs.
Understand the services we provide
We will share clear information about our services, including the Patient Management Program—its goals, features, and any limitations, changes, or discontinuation—whenever you request it.
Receive information about your medication
Clinical pharmacists are available 24 hours a day to answer questions. You will also receive written drug information with your prescription. You have the right to speak with a healthcare professional whenever you need support.
Know who is caring for you
Each time you contact the pharmacy, our team will provide their name and job title. If you request it, we will connect you with a supervisor or another healthcare professional.
Expect privacy
Your medical information—shared through conversations, messages, or your medical record—is kept confidential as required by state and federal law. You or your approved representative may access your health information as allowed by law. Personal health information is shared with the Patient Management Program only in accordance with privacy regulations.
Participate in your care plan
You may take part in creating and updating your care plan. We encourage you to ask questions about your treatment, medication therapy, or the Patient Management Program at any time.
Understand medication costs and payment options
Before we fill your prescription, we will review your medication cost and payment options with you. When possible, we will help you apply for available assistance programs.
Make choices about your care
You may decline participation, revoke consent, or withdraw from the Patient Management Program at any time. You may also choose your own prescriber and preferred pharmacy providers.

If you have a concern or complaint, you may call 513-585-9700 or submit a patient complaint form included in your welcome packet or online at uchealth.com/pharmacy/specialty.
