What It Is

Connected Care takes the care you get from a UC Health provider and brings it into your everyday life. 

If you enroll, you will have acces to a care team that can help support your health. You'll receive educational materials, help coordinating your care and qualified experts to walk you through it all.

The Connected Care team works with your UC Health provider and does not replace your primary or specialty care.

How the Program Works

You may be identified for UC Health Connected Care based on your health needs and your provider’s recommendation.

If you’re eligible, you may receive information about the program through MyChart, email, phone outreach or other communications.

If you enroll, the Connected Care team will reach out to provide support and periodic check-ins based on your care needs. 

 

What to Expect

The Connected Care team may help with:

  • Understanding and managing medications

  • Coordinating referrals, lab tests and preventive screenings 

  • Providing education on diet, exercise and overall wellness

  • Helping identify community and support resourcesShape

     

Remote Patient Monitoring (If Eligible) 

Some patients may qualify for remote patient monitoring services as part of Connected Care.

If applicable, you may receive an FDA-cleared device to use at home, such as a blood pressure monitor, glucose monitor or scale. These devices allow you to track certain health measurements.

The Connected Care team may review your information and follow up with care recommendations.

  • Stay in tune with your health plan between visits

  • Better understand and manage your health conditions 

  • Receive additional support from your care team

Individual experiences may vary. Connected Care is not intended for emergency situations. If you are experiencing a medical emergency, call 911.

 

Cost and Coverage 

UC Health Connected Care is a Medicare-covered service.

Depending on your insurance plan, you may have cost-sharing responsibilities, such as coinsurance or copayments. Many patients have little to no out-of-pocket cost after meeting their deductible. A member of the care team will review any potential costs with you before you enroll.

Participation and Privacy 

Participation in UC Health Connected Care is voluntary. You may choose whether to enroll and may opt out of the program at any time.
