How to Access the Epilepsy Transition Clinic

Moving from pediatric to adult epilepsy care is a big step. We’ll guide you—at your pace—with clear next steps and a team that knows you.


 

How to Access the Clinic

Please tell your pediatric neurology provider you’re interested in the Epilepsy Transition Clinic before your final pediatric visit. After your provider places a referral, our team will contact you to schedule.

If you don’t hear from adult scheduling within two weeks of your final pediatric visit, call 513-475-8730 to make an appointment.

Since opening the clinic, we’ve successfully transitioned more than 150 patients to adult epilepsy care. Your plan is tailored to your needs and goals.

 

Transition Preparation

When Does Transition Begin?

We recommend starting transition talks around ages 16–17. Most teens move to adult care between 18–21. Your plan will be based on your needs and abilities. Some patients will need more caregiver support. If you and your pediatric team decide you no longer need seizure medicine and remain seizure-free, you may not need ongoing adult neurology care.

How You Can Prepare

  • Practice seeing your doctor on your own, if possible.
  • Learn skills to manage your epilepsy (medications, triggers, seizure plan).
  • Keep a health passport or summary with your medical history, medications, allergies and emergency plan for your new adult doctor.
  • Make a step-by-step plan to transfer to adult care.

What to Expect at Your Transition Visit

At your first visit in the transition clinic, you’ll be welcomed by the Epilepsy Alliance of Ohio and an adult neurologist. We’ll review your history, answer questions and outline your next steps.

What to Bring to Your First Adult Visit

  • Your medical information or health passport for your new doctor.
  • Photo ID and health insurance card.
  • A written list of questions or concerns. Your new doctors are happy to help.
  • You’re always welcome to bring a parent, caregiver or friend.

Frequently Asked Questions

How long will my first visit last?

Your first visit may last up to two hours. This includes time with your doctor and a social worker so we can understand your needs and plan your care.

What will I be asked?

Your team will ask about the types of seizures you have, how often they occur, your current and past treatments and your goals for school, work and life. This helps us build a plan that fits you.

What if I need a refill before my first UC Health appointment?

Until your first appointment with UC Health, please continue to contact your pediatric providers for prescription refills and questions.
