Patients may be referred to our Pharmacist Care Clinics and Anticoagulation Services by their providers for various reasons, including medication therapy monitoring, individualized mediation therapy counseling and disease state management.
Most patients are eligible for our services, but a provider referral is required. Please inform your provider if you are interested in a referral to one of our UC Health Ambulatory Care Clinical Pharmacy Services. While most insurance plans cover our services, a copayment may be required.
