How to Prepare for Your Primary Care Appointment

Preparing for your visit helps you get the most out of your time with your primary care provider. Use this guide to understand what to bring, when to arrive, and how to schedule or reschedule appointments.


Scheduling Routine and Urgent Appointments

Routine check-ups, physicals, and follow-up visits should be scheduled during regular office hours. Booking early improves your chances of getting the day and time you prefer.

If you need care quickly, we make every effort to offer urgent appointments within 24–48 hours, depending on your needs. If your primary care provider is unavailable, another qualified clinician will see you to ensure you receive timely care.

To help us stay on schedule, please note that arriving 15 minutes or more late may require rescheduling.

Please note that same-day and urgent appointments are limited and scheduled based on medical need and availability. Call 911 or go to the nearest emergency room for life-threatening symptoms such as chest pain, difficulty breathing or stroke symptoms.

 

What to Bring to Your Appointment

Bringing the right information helps your care team understand your health needs:

  • A photo ID

  • Your insurance card

  • Medication list with dosages and instructions

  • Any recent test results or medical records, if applicable

  • A list of questions or concerns you want to discuss

Having these items ready helps us personalize your visit and keeps check-in fast.

 

When You Arrive

We ask that you arrive 10 minutes before your appointment to complete or update your medical information. Copayments are collected at check-in.

If you take medications, bring a complete list or the bottles. This ensures your provider has accurate information for safe treatment. Writing down your questions can also help you get the most out of your visit.

 

Canceling or Rescheduling an Appointment

If you need to cancel, please contact us at least 24 hours in advance. This allows us to offer the appointment time to another patient with urgent needs.

Patients who miss three or more appointments without notice may be dismissed from the practice.
