Care Delivered Directly on the Street and in Encampments
Our team meets people in encampments and on the street so they don't have to travel to a clinic to get help. This street-based care is designed for people experiencing homelessness who face barriers like transportation, stigma or fear of traditional settings. We focus on building trust over time so patients can get the medical, behavioral health and addiction care they choose, at their own pace.

Nonjudgmental Support for Substance Use Treatment
Many of the people we serve live with opioid use or other substance use disorders. Our team offers harm reduction supplies, naloxone and substance use treatment options without judgment or pressure to stop before patients are ready. When patients choose treatment, we help them create a personalized plan and connect them to ongoing addiction and primary care through UC Health and our community partners.

Connection to Ongoing Primary and Preventive Care Supports Recovery
Street visits are often the first step. From there, we link patients to primary and preventive care so they can manage chronic conditions and stay ahead of new health problems. Our team helps bridge people from the street to ongoing care, including follow-up for diabetes, wound care and other long-term needs, through UC Health and community partners.

Our program is a partnership started by a grant from the OneOhio Recovery Foundation, bringing together the UC Department of Family and Community Medicine and Emergency Medicine (UC Health Physicians), the UC Early Intervention Program, Greater Cincinnati Behavioral Health and NeighborHub Health Federally Qualified Health Center.
