How to Transfer Prescriptions

Transfering your prescriptions to UC Health Retail Pharmacy means our pharmacists can support your ongoing care, manage refills and answer questions about your medication and health conditions.

Here’s how to transfer your prescriptions

Gather your information:

• Your full name, date of birth and contact details

• Pharmacy name and phone number

• Prescription name and numbers needing transferred

Contact UC Health Pharmacy:

• MyChart: Log in to MyChart and send a message requesting a transfer, or submit your information through your account.
• Phone: Call one of our Pharmacy Locations and provide your prescription and pharmacy details to our pharmacy team to initiate a transfer.
• Form: Submit a Transfer Request

We’ll handle the rest:

UC Health pharmacists will contact your previous pharmacy, transfer your prescriptions and let you know when your medications are ready for pickup or delivery either through text or MyChart.

Common Questions About Prescription Transfers

Can all prescriptions be transferred?

Most medications can be transferred, but some prescriptions (like controlled substances or specialty medications) may have additional requirements. Our team will walk you through any extra steps or reach out in the event that a prescription cannot be transferred.

Will my refills transfer too?

Yes, refills are included as part of your prescription so in most cases your remaining refills transfer along with your prescription.

Do I need to contact my doctor?

Usually, no—UC Health will coordinate with your provider as needed to ensure continuity of care.

What if I have questions about my medication or health conditions?

Our pharmacy team is here to help. We can review your prescription history, discuss your conditions and answer any questions about your medications or possible side effects.

How long will the transfer process take?

Most transfers are completed within one – two business days. Certain prescriptions (such as controlled substances) or special conditions may take longer or may not be transferred at all—our team will keep you informed.
