Body mass index (BMI) is a number calculated from your age, weight and height that helps healthcare providers diagnose obesity.  Your BMI will help determine which weight loss program is right for you. Enter your information in the calculator to find your BMI.


  • Normal BMI (0-26.99): You do not qualify for a weight management program
  • Above Normal BMI (27+): You may qualify for our non-surgical weight management program
  • High BMI (35+): You may qualify for our surgical and non-surgical weight management programs

All new non-surgical weight loss candidates must complete a free informational seminar prior to enrolling in our program.

To register for our non-surgical weight loss seminar, you must be between the ages of 18 and 64.

Step 2: Register to Attend

Please note: Once you submit your registration, you will be sent a link to the email address you provided so that you can access your online seminar video. You must view the entire video to proceed to the next steps in your weight loss journey.

Step 3: Submit a Patient Medical History Form

New patients must download and submit a patient medical history form.

Please complete and submit the form, along with a front and back copy of your insurance card. You can submit this information using one of the following options:

Step 4: Thank you

After you have submitted your paperwork and attended the free informational seminar, a designated patient advocate will contact you to schedule your initial consultation and enroll you into our program.

Step 4: Final Steps to Begin Your Journey

After you complete your paperwork and complete the surgical seminar, our insurance specialist will contact you within 7-10 business days to talk with you about your insurance coverage or financing options.

Our program accepts a variety of health insurance plans, but coverage varies widely. We will help you determine what your policy covers.

After insurance verification, you will be scheduled for your medical assessment with our care team. A patient advocate will provide you with all the information you need before your first appointment.
