Q: What is the easiest way to refer a patient?
A: The easiest way to make a referral is to complete and submit a referral online, or download, print and complete a referral form and fax to 513.418.2599.

Q: Does Daniel Drake Center for Post-Acute Care accept patients without insurance?
A:
Yes, Daniel Drake Center will accept a patient without insurance; however, there are financial criteria that must be met in order for the patient to be cleared and approved for admission.

We offer a 40 percent discount to patients who are paying out of pocket. For patients who are unable to pay their bills, we can help identify resources for financial assistance.

Our onsite eligibility specialists are available to help patients apply for benefits like Social Security Disability, Supplemental Security Income and Ohio Medicaid.

Q: Is Daniel Drake Center in-network with my patient’s insurance?
A: Daniel Drake Center is in-network with most of the insurance plans we accept; however, there are a few plans we accept for which Daniel Drake Center is out-of-network. Insurance plans accepted by Daniel Drake Center.

Once you have referred your patient, one of our Assessment Liasions will take care of contacting your patient’s insurance carrier to verify benefits, determine the need for additional assistance and obtain approval for the transfer to Daniel Drake Center.

Q: Do I need to call my patient’s insurance company to obtain approval for a transfer to Daniel Drake Center?
A: No. Our comprehensive referral process includes pre-certification for insurance approval. Once you have referred your patient, one of our Assessment Liaisons will contact the patient’s insurance carrier to verify benefits, determine the need for additional assistance and obtain approval for a transfer to Daniel Drake Center.

Q: Can my patient’s attending physician or primary care physician follow him/her at Daniel Drake Center once he/she has been admitted?
A: Physicians can follow their patients only if they have staff privileges at Daniel Drake Center. Every long-term acute care patient is assigned a staff of experts assembled to meet their unique goals and diagnosis. Daniel Drake Center’s medical staff includes hospitalists, and physician specialists in pulmonology, cardiology, infectious diseases, nephrology and other fields.
